How to Get UMID ID Card

It was in 2010 when the Unified Multi-purpose Identification (UMID) card was introduced, signed by President Gloria Macapagal-Arroyo under Executive Order No. 420. As part of the Phillippine government’s efforts to establish a national identification document, this card was released to streamline identification systems nationwide. Government agencies responsible for implementing UMIDs are Government Service Insurance System or GSIS, Social Security System or SSS, PhilHealth or Philippine Health Insurance Corporation, and PAGIBIG Fund. 

 

This card features a UMID bearer’s face, signature, common reference number (CRN), and a “Republic of the Philippines Unified Multipurpose ID text logo on the top portion. You can also see the logo of PhilHealth, PAGIBIG, SSS, and GSIS at the back of the ID. It also includes details of the bearer’s surname, middle name, first name, birth date, sex, and address.

 

The UMID Cards can be used for most transactions, including opening a bank account and getting a house and lot in the Philippines. So, if you don’t have a UMID card, here are our guidelines for getting one.

 

 

How to Get Your SSS UMID Card

Before applying for an SSS UMID card, you must check first if you are eligible to get one. You must be an SSS member with at least one (1) posted contribution. You can also apply if you lose your SSS ID or need to change your details.

 

 

SSS UMID ID Requirements

You must submit a duly accomplished UMID application form and at least one (1) valid government-issued ID or two (2) secondary documents bearing your identification details. You must also submit an affidavit of loss for a lost UMID card. 

 

 

How to Apply for SSS UMID Card

You have two (2) options to apply. You can visit your preferred SSS branch office for walk-in transactions. But be sure to visit based on your transaction date. SSS follows the strict implementation of a number coding scheme

 

On the other hand, you can also set an appointment for the UMID application via My. SSS appointment system. Don't forget to bring a printed SS number application and transaction number slip.

 

Whether it is online or walk-in, here are steps in getting your UMID card.

 

 

Bring the UMID Card Application form to the nearest SSS branch office.

Prepare the documents needed along with the UMID Application Form before going to the nearest SSS branches. Get your priority number at the information desk and wait for your turn to be called. Be sure you bring original copies of your documents for verification. A designated SSS officer will interview and evaluate your documents once you are called.

Once checked, you will be asked to pay the UMID fee of P200.00 for card replacement or lost ID. But you don’t have to pay anything if you’re a first-time user.

 

 

Proceed to Biometrics Area.

Head to the biometrics photo booth to have your biometrics taken. This includes your photo, fingerprints, and digital signature. Review the details before leaving the capture area to ensure all the information is correct. After confirmation, you will be given a claim stub or acknowledgment receipt that you have completed the UMID card application.

 

 

Wait for the release of your UMID card.

Releasing your SSS UMID card will be indicated on the claim stub. The assigned SSS officer will also inform the date of release. Also, you can check the status of your UMID via My. SSS account portal. Go to the Member Info drop-down menu and click the SSS/UMID ID Details button.

 

 

Important Note

The SSS UMID Card has the option to serve as your ATM card. You can visit the nearest UnionBank to upgrade your UMID. Click the Data Sharing Consent for UMID Card Upgrade on the My.SSS Services menu. Then, download the UnionBank mobile app or visit their log-in menu to confirm the UMID upgrade. You can visit the bank for the release of your upgraded UMID card or have it delivered to your home address.

Your SSS UMID Card will also serve as your savings account and a disbursement channel for SSS benefits.

 

 

How to Get Your GSIS UMID Card

The GSIS UMID Card has a three-in-one feature, serving as your valid ID, ATM card, and debit card. It has an RFID microchip embedded in the card containing all your information. You can also use your GSIS UMID card to access the GW@PS kiosk for any GSIS services.

 

 

GSIS UMID ID Requirements

All GSIS active members are welcome to apply for a UMID card. The requirements are a duly accomplished GSIS UMID Card Application form and at least two (2) valid IDs.  

 

 

How to Apply for GSIS UMID Card

Applying for GSIS UMID Card is similar to the SSS UMID Card application. Here’s how.

 

 

Download and print the application form.

Go to the GSIS official website, download, and print the application form. Fill up the required details and ensure all the information is accurate. 

 

 

Visit the nearest Government Service Insurance System office.

Bring the required documents when visiting the GSIS handling branch with a UMID enrollment facility. Proceed to the biometric data capture area to take your photo, digital signature, and fingerprints. A GSIS officer will review your documents and give you an acknowledgment receipt indicating you have completed the application.

 

 

Wait for the release of your GSIS UMID card.

The assigned GSIS officer will tell you the pick-up date of your UMID card. Present valid government-issued IDs upon claiming.

 

 

Activate your UMID Card.

Insert your UMID card in the GW@PS kiosk and tap the Activate UMID Card button. Once activated, you can now use your UMID Card to receive GSIS loans and benefits. You can also use this card as your personal ATM savings account or debit card.
